Insurance billing can be complicated and requires cooperation between doctors, patients and insurance providers to meet the requirements needed to assist with the reimbursement of your home medical products and prescribed medications. Our Insurance Billing team knows and understands insurance-billing requirements (including Medicare and Medicaid) and can help bill your claim and answer your questions.

To expedite your insurance process prior to coming to our store location, please print, complete, sign and bring all required items listed below to fulfill proper billing requirements needed by your insurance company. For all insurance companies the following forms or documents are required:
- A valid prescription for the item(s) you need
- A copy of your Insurance Card
- A copy of your current Driver's License or other appropriate ID
- Order Intake Form
- Customer Information Checklist
- Insurance Verification
